1. I have previously been to another chiropractor for treatment. Do I still need to schedule an initial exam?
If you are seeking chiropractic treatment for the first time or have not been seen for 6 months or longer, please schedule a new patient exam for your first appointment.
2. I do not see anything about insurance on your website, do you accept health insurance?
In order to provide you with the best care possible, we do not currently accept or work with any health insurance providers. However, in the case that your visit is eligible for independent reimbursement by your insurance provider, we will provide you with a properly coded bill of services.
3. How do I know if Functional Medicine is right for me?
If you are seeking a more holistic health approach, Functional Medicine may be the solution. Dr. Zimmerman offers a FREE 15-minute phone consultation to make sure you are a good fit prior to scheduling your first appointment. If you would rather schedule your initial appointment right away, then please call the office to schedule your initial functional medicine appointment.
4. Are there any forms that I need to fill out prior to my initial exam?
Upon scheduling your initial exam, you will receive all of the paperwork that needs to be filled out and signed before your exam. If possible, please come to your first appointment with all of your paperwork filled out and ready for Dr. Zimmerman to review prior to your appointment. Her questionnaires are quite lengthy and can potentially take a lot of time to fill out. Please also bring with you any previous blood/lab work results, as well as a comprehensive list of any medications or supplements you are currently taking.
